1. Complete the online enrollment form and sign the necessary waivers HERE, if you haven’t already.
2. Go HERE to make your PLEX Hockey Summer Camp week selection and pay.
NOTE: Until you complete the payment process and select your participating weeks, your spot is NOT guaranteed. It is important to do this right away after completing the enrollment form.
3. Download the necessary forms below for completion and return to the Program Director, or submit via email.
